8 Weeks to Vitality
This coaching package is specifically designed for individuals with type 2 diabetes or those who are pre-diabetic. The program aims to help clients stay compliant with their doctor's recommendations and achieve their health goals.
Week 1: Setting the Foundation
Purpose: To understand the client's medical history, lifestyle, and specific doctor recommendations
What We Do: Discuss medical background, current health status, and doctor's recommendations, identify client's health goals and challenges, create a personalized action plan.
Home Focus: Keep a detailed log of daily activities, meals, blood sugar levels, and any symptoms.
Week 3 Topic:
Physical Activity and
Body Movement.
Goal: Incorporate appropriate easy and fun physical activity into the client’s daily routine.
What We Do: Assess current physical activity levels and discuss its benefits for diabetes management, create a customized physical activity plan, based on doctor recommendations, discuss ways to stay motivated and overcome barriers to physical activity.
Home Focus: Follow the physical activity plans and record physical activities and their effects on blood sugar.
Week 5: Stress Management and Emotional Well-being
Purpose: Address stress and emotional factors that impact diabetes management.
What We Do: Identify stressors and their impact on blood sugar, introduce stress management techniques such as mindfulness, meditation, and relaxation exercises. I will help you find what methods resonate with you, discuss emotional challenges related to living with diabetes.
Home Focus: Practice stress management techniques daily and track emotional responses and their effects on blood sugar. Email me a list of their medications in preparation for next session.
Session 7: Problem Solving and Overcoming Barriers
Purpose: Resolve challenges and barriers to staying compliant with health goals.
What We Do: Review progress and identify any challenges or barriers encountered, help you create problem-solving strategies for common issues such as dietary temptations, exercise avoidance, or medication non-compliance, help you create a plan to address each barrier and set short-term goals.
Home Focus: Implement the strategies discussed and monitor their effectiveness.
Week 2: Nutritional Guidance Based on Your Doctor’s Recommendations.
Purpose: Deeper understanding of proper nutrition
tailored for diabetes management.
What We Do: Review meal and snack logs from last week and identify areas for improvement, discuss the importance of carbohydrates, glycemic index, and portion control, help you develop a meal plan that aligns with doctor recommendations.
Homework: Implement the meal plan and track food intake and blood sugar levels.
Week 4: Monitoring and Managing Blood Sugar
Goal: Enhance understanding and management of blood sugar levels.
What We Do: Review blood sugar logs and discuss patterns and triggers, discuss how different foods, activities, and medications affect blood sugar.
Home Focus: Continue to monitor blood sugar and note any patterns or significant changes.
Session 6: Medication
and Compliance
Purpose: Ensure understanding and adherence to prescribed medications.
What We Do: Review current medications, their purposes, and potential side effects, discuss the importance of medication compliance and strategies to stay on track, help you create a medication schedule or checklist.
Home focus: Follow the medication schedule and report any issues or side effects.
Week 8: Celebrate Progress and Future Planning
Objective: Review progress, celebrate achievements, and plan for long-term success.
What We Do: Summarize progress made over the 8 sessions and celebrate successes, review and refine the action plan for ongoing health management, set long-term goals and discuss strategies for maintaining momentum and addressing future challenges, discuss options for further support.
Home Focus: Continue to follow the action plan and schedule regular follow-up sessions to ensure ongoing support.
